Zuri Health Launches Continuous Care Programme
mHealth app Zuri Health has launched its Continuous Care Programme. Designed to offer support to persons with chronic illnesses, the programme will be driven in partnership with Pharmaplus Pharmacies Limited, a Kenyan brand with over 50 outlets dotting the country. This new service helps people, especially those with chronic health conditions like hypertension or diabetes, keep up with their healthcare demands.
This collaboration aims to transform chronic care patients’ lives by offering them a wider range of healthcare services, including medication management and access to the Continuous Care Programme. “People with chronic illnesses have complex health needs that current healthcare delivery systems do not address or provide for in a seamless manner.
